A Six Month Treatment Package
Lifestyle Medicine Specialists offers 6-month prepaid packages for for Hormone Replacement Therapy. During the program, Lifestyle Medicine Specialists will provides medical services and treatment related to Hormone Replacement Therapy only. Patients that elect to the Hormone Replacement Program will need to continue care with their primary care physician for all other general medical needs.
Cost for Hormone Replacement Therapy
The prepaid package fee includes office visits only and does not include the cost of medications and supplements, or laboratory/diagnostic testing which may be covered by your insurance.
What to Expect
Initial Consultation
Your initial office visit will include a review of your medical history including medical records, if available, and a physical examination. Following your initial office visit, appropriate laboratory/diagnostic testing may be ordered. During your second office visit, your laboratory/diagnostic test results will be reviewed and a physical examination and assessment will be performed following which a treatment plan will be established. Any necessary prescriptions and/or referrals for consultations will be made, if necessary.
Follow-up Office Visits
Follow-up visits will be scheduled as necessary to review medical progress, among other things. Your provider will decide on the appropriate interval of follow ups. You should expect to have your initial and second office visit during the first 2-week interval depending on medical records and laboratory/diagnostic testing needed. Follow-up appointments will be scheduled at 2-to-4- week intervals moving forward depending on your individual progress. Follow-up appointments will be scheduled for a mutually convenient time for you and the provider at the conclusion of each visit. Generally speaking, patients are seen approximately 6 to 7 times in a 6-month period, but can vary depending on individual patient progress.
Optional Continuation of Hormone Replacement Therapy Treatments
Should you choose to elect to continue treatment after the expiration of the initial 6-month package, the renewal fee must be paid prior to continuing treatment.

What are your normal office hours?
Appointments are generally available Monday through Friday from 8:30 AM to 2:00PM. To schedule an initial evaluation or treatment, please call (561) 318-3727. On occasion, Lifestyle Medicine Specialists may be able to accommodate appointments outside these normal hours.
